In partnership with the Y4 class teacher:

• Compile pupil tracker information for the following academic year (Year 5), based on end of Year 4 outcomes
• Analyse teacher assessments, children's work and optional QCA assessments to identify key aspects of literacy and mathematics that need to be addressed for specific groups
• Map out possible use of Wave 2 and Wave 3 interventions for the next academic year
• Plan and deliver units to support effective transition to next year group/school
