In partnership with the Y6 class teacher:

• Use teacher assessments and optional test results to update pupil tracker information with end-of-year outcomes.
• Discuss actual NC progress of groups and individuals with Year 6 class teacher.
• Share analysis of teacher assessments, children's work and optional QCA assessments and identify key aspects of literacy and mathematics that need to be addressed for specific groups.
• Discuss possible use of Wave 2 and Wave 3 interventions for the next academic year.
• Plan and deliver units to support effective transition to next year group/school.
